Great job. This is a very informative survey and I'm sure that it must have helped you. Some of these questions however, are either not opinion questions, and really don't hold that much value. I mean, if I thought lung cancer was the worst effect of smoking, would that matter ? I mean, I could have selected bad breath or heart disease too. What would the difference be? Anyway, it's a well developed form and great job.
